Justice360° - Name Change and Renewing Passport

 

If you have legally changed your name, you are required to renew your passport. There are different procedures for renewing a passport, depending on the date your passport was issued and why you changed your name:

 

 

Passport issued less than a year ago

 

And

 

Name legally changed due to marriageor court order

 

 

 

Steps:

 

1.      Complete Form DS-5504 (application for a U.S. passport: name change, data correction, and limited passport book replacement) (link and specific steps for the form: http://www.travel.state.gov/ passport/forms/ds5504/ds5504_ 2663.html)

 

2.      Mail the following documents with the DS-5504 form on the address listed on the form:

a.       Your current, valid U.S. passport

b.      An original or certified copy of your marriage certificate or the government-issued document evidencing your legal name change under federal or state law.

c.       If passport was issued more than 90 days ago, one new color passport photo

 

Note:

 

·                There is no fee for this services unless you require expedited service (link: http://www.travel.state.gov/ passport/hurry/hurry_831.html )

·                It is strongly encouraged to mail passport application and any personal documents using a traceable delivery method and to use a secure means of packaging such as Tyvek envelope

 

 

Passport issued more than a year ago

 

And

 

Name legally changed due to marriage or court order

 

Steps:

 

1.      Complete Form DS-82 (application for a U.S. passport by mail) (link and specific steps for the form: http://www.travel.state.gov/ passport/forms/ds82/ds82_843. html )

 

2.      Mail the following documents with the DS-82 form on the address listed on the form:

a.       Your current, valid U.S. passport

b.      An original or certified copy of your marriage certificate or the government-issued document evidencing your legal name change under federal or state law.

c.       One new color passport photo One recent, color passport photograph

 

3.      Pay applicable "renewal" fees (link:http://www.travel.state.gov/ passport/fees/fees_837.html )

 

Note:

 

·                Minors may NOT apply using Form DS-82 and should refer to the instruction below.

·                It is strongly encouraged to mail passport application and any personal documents using a traceable delivery method and to use a secure means of packaging such as Tyvek envelope

 

 

If you cannot legally document your name change (through court order or marriage)

 

Or

 

If you are a minor under the age 16 whose passport was issued more than one year ago (specific rules for all minors under age 16:http://www.travel.state.gov/ passport/get/minors/minors_ 834.html )

 

Steps:

 

1.      Complete Form DS-11 (Application for a U.S. Passport) (links and specific steps for the form: http://www.travel.state.gov/ passport/forms/ds11/ds11_842. html )

 

2.      Mail the following documents with the DS-82 form on the address listed on the form:

a.       Your current, valid U.S. passport

b.      At least 3 public records showing your date and place of birth that show exclusive use of your assumed name for at least 5 years

c.       One recent, color passport photograph

3.      Pay applicable "first time" fees (link:http://www.travel.state.gov/ passport/fees/fees_837.html )
